Contribution Request Form

The Mortenson Charitable Foundation (MCF) is committed to programs that support education and health care in our community and around the world, and to support our team members and their families in times of crisis and educational endeavors. A special emphasis is placed on children where possible in all areas that we support.

For grant donation requests, please complete and submit this form with supporting materials. Applications will be reviewed by the Grants Committee and voted on at the next quarterly MCF meeting. A written response will be submitted when a decision is made. Please do not apply more than once in a given year. Donations will only be given to registered 501(c)(3) organizations. Contribution Guidelines

Contributions will only be given to registered 501(c)(3) organizations.
Requests should be for charities that support or are related to education, general health, or healthcare, especially where children are a direct or indirect beneficiary. Requests for support outside of these areas will generally not be considered.
If the contribution involves a deadline, the request must be submitted at least 30 days in advance of when funds are needed in order to be considered.
An organization can only receive one MCF contribution within a calendar year.
Documentation: All requests must include the following:
Mortenson Employees: Include a list of any Mortenson employees who currently serve on the board or actively volunteer for the organization.
Reports: The organization’s most recent annual report, audited financial statements and operating budget (including a summary of administrative costs).

If a 501(c)(3), attach a copy of IRS certification granting tax-exempt status. If not a 501(c)(3), provide any relevant documents from the organization.
